CMS Short Cycle Regulations by January 1, 2013.
As a result of the healthcare reform legislation that was passed in 2010, Congress mandated CMS to reduce the amount of medication waste. They have mandated a 7 days or shorter cycle fill on all oral solid and liquid brand medications. Read more about the proposed changes, and other requirements and restrictions.
At Grane Rx, we are excited about this new change. It will provide our clients with the opportunity to improve efficiencies, increase compliance, and save money.
State and Federal Surveys
Survey deficiencies can lead to bans on admissions, monetary penalties and lower Five Star Quality ratings. Complete and accurate documentation is imperative to survive these surveys and receive high quality score.
The Grane inSTAT Rx™ Solution will provide the safety and accuracy that surveyors demand from LTC facilities. Our easy-to-use interface is designed specifically for nurses. This includes medications in resident specific pouches that are clearly labeled and easy to read. All medications that are dispensed either on site at the facility or in the pharmacy are electronically documented which provides for an easy to read audit trail for surveyors.
Staffing Efficiencies
Families and residents expect the best care from nurses and facility staff. As the industry becomes more complex and acute, demands for nurses' time will continue to increase.
Our inSTAT Rx™ Solution will improve medication pass times so nurses and staff can spend more time focused on resident care. Less time is focused on finding medications, administering medications from outdated blister punch cards, and chasing down missing paperwork. Find out how our inSTAT Rx Solution can solve your staffing issues.
The Grane inSTAT Rx™ Solution is truly one of the most innovative and cutting edge devices offered in the long term care setting. It is the 1st on-demand, medication distribution packager designed by nursing home operators and pharmacists. This was designed purposely to maximize the clinical, financial, operational, and regulatory benefits so that all parties involved in the process could experience the benefits.
Order Medications
Medications are ordered by nurses through electronic order entry or electronic fax transmission. Our order entry team will review, edit, and enter the order. Then, verify insurance and recommend any therapeutic interchange if necessary.
Pharmacist Review & Verification
Our team of pharmacists will review all medication orders and perform clinical screening, then immediately send the order to be dispensed through the inSTAT Rx™ device located at the facility.
Medications Packaged On Site or In the Pharmacy
On Site: When the nurses are ready to process the medications, the nurse will follow the easy step by step instructions. This interface was built by nurses for nurses to ensure simplicity and accuracy. Once the medications are processed, they can be administered to the patient. InSTAT Rx packages most oral solid medications; non-controlled and controlled, routine and PRN.
In the Pharmacy: Medications will be dispensed in the multi-dose and unit dose medpacs at the pharmacy. We will then deliver the medications a in a timely and accurate 4/3 medication quantities to your facility. This process ensures that your patients have the right drug at the right time and limits your organization to unnecessary costs and administrative labor associated with wasted medications.
Nurse Administers Medication
With the easy to read and review medpacs, nurses do not have to handle the bingo/blister cards over and over. These patient-specific medpacs are clearly labeled with the patient's name and medication information, which can include bar-coding capabilities to interface with your eMAR.
Once you have reviewed the medications, simply tear open the medpac and administer the medication to the resident.
Manage, Monitor, and Report
The inSTAT Rx™ system is designed to provide detailed reports that are vital to your nursing team. Permission-based biometric finger print scanning security ensures that only authorized personnel can access these reports.

When identifying the best solutions for short cycle medication dispensing, Grane Rx wanted to maximize the amount of savings that a facility could expect. The savings can be substantial in a number of different areas, including:
90% Reduction in Medication Waste
By providing shorter cycle dispensing through on site medication dispensing or in the pharmacy dispensing, facilities will immediately see a reduction in medication waste. This can add up to tens of thousands of dollars in savings. An internal study completed by a large nursing home chain demonstrated a potential for 10% reduction in medication costs per facility.
Formulary Management
Many formulary issues go unnoticed by nursing homes with results in higher drug costs. With the Grane Rx inSTAT Rxª Solution, when medications are dispensed on site at the facility, each packager is specifically calibrated to maximize the formulary specific to your needs.
Reduce Patient per Day Drug Expense
With the on site inSTAT Rx Solution, you can choose to package medications either 24 hours in advance or right before each medication pass to ensure that you dispense current medication orders, including dose changes, and do not dispense the discontinued medications. When the medications are packaged in the pharmacy, inSTAT Rx is focused on improving these expenses as well.
Improved Cash Flow
A short cycle dispensing method means fewer inventories on hand, allocating cash flow for other areas of your business.
Therapeutic Interchange Program
The Grane Rx Therapeutic Interchange Program is designed to improve resident outcomes and minimize cost to the skilled nursing facility.

Grane Rx is determined to assist doctors and nurses in providing more positive outcomes for their residents. Grane Rx can effectively combine the latest technology advances with formulary management, efficient medication administration and electronic documentation.
Medication Availability for Admissions Anytime of the Day
One of the biggest factors in improving resident outcomes involves improving medication availability. Many nursing facilities are dealing with later and later admissions from hospitals that are past the pharmacy cut off times for ordering of medications. This can present a number of challenges.
The on site inSTAT Rx Solution virtually eliminates this problem because oral solid medications are dispensed remotely, onsite at the facility, in patient-specific multi-dose and unit dose packaging. All orders are reviewed by a licensed pharmacist prior to medications being dispensed.
Response to Medication Changes and Formulary Management
The ever increasing patient acuity that facilities and staff continue to monitor results in frequent changes in medications and having the ability to respond quickly to these changes is a critical component in having positive outcomes for residents.
Our Complete Pharmacy Solution minimizes that lag time and streamlines the formulary management process. Since the nursing staff will know what medications are in the inSTAT Rx™ device, they can immediately respond to the physicians order or medication changes.
Reduction in Medication Errors
Safety is always everyone's top priority. The inSTAT Rx™ Solution dispenses medications from a secure machine. The medications are sealed in pharmacist approved tamper resistant containers. Medications are then packaged from the remote tablet packager into patient specific unit and multi-dose medpacs. Nurses open the medpacs and administer medications to the resident similar to what they do today. Less handling of medications reduces the likelihood of medication administration mistakes.
